Nominate A Potential Recipient

Nominations will be reviewed on a case by case basis. Potential recipients must be determined to have been directly or indirectly affected by a cancer diagnosis and be in the position to receive assistance due to financial hardships resulting from the cancer diagnosis. The Swing Fore Hope Board of Directors will review each nomination and determine eligibility for financial assistance and whether the potential recipient falls within the foundation’s mission of financially assisting individuals and families in need of financial assistance.
